TeamWell Health is happy to offer direct billing to all eligible patients at some of our locations!  Payments for your therapy goes directly to the clinic, saving you the trouble of dealing with claiming your money back.

Select plans cover the entire cost of therapy in full! Others may require a small co-payment.

To find out if you are eligible, simply contact us and provide the following information:

1.  Your Name and Birthdate  (Name and birthdate of partner/spouse/parent if Insurance plan is under their name)

2.  Insurance Company Name

3.  Plan or Policy #

4.  Member or Certificate #
